Registration forms must be down-loaded, filled out completely and returned with your registration by Sept. 20^th. The five forms are:

1.    USA HOCKEY reg. bar code (unless submitted w/ pre-reg)

2.    USA Hockey Code of Conduct

3.    USA Hockey Consent to Treat

4.    GGHO Code of Conduct

5.    GGHO Registration

Mail all forms and payment to our PO Box shown above or deliver them in person at the rink on Sept. 4^th during our used equipment sale from 10:00am-1:00pm. If you cannot access the forms via the website printed forms will be available at the rink. Half of the registration fee is due SEPTEMBER 20th and the remaining amount MUST be paid by DECEMBER 6^th. Families with 2 or more players in the organization are eligible for a discount of 10% of their total registration fee.

Those players with an outstanding balance on December 6th WILL NOT be allowed on the ice until the balance is received.
